Health systems around the world are struggling with rising patient demand and Middlemore Hospital, which services a growing and ageing population, is no exception.
Building bigger hospitals is not an affordable or sustainable solution so Counties Manukau Health has been working with health professionals across the sector to reduce the demand on hospital services through the 20,000 Days campaign.
The aim of the campaign was to give back 20,000 well and healthy days to the community by anticipating and preventing acute health problems, responding quickly and effectively in the community, and providing timely and safe care to people admitted to hospital.
On 2nd July 2013, campaign team members, patients and their families gathered at Ko Awatea to celebrate their achievement of giving back 22,940 healthy and well days for the people served by CM Health. It’s been an amazing journey – watch the video below.
13 collaboratives took part in the campaign, and a number of initiatives, such as the Healthy Hearts Programme, Better Breathing Programme, VHIU (Very High Intensity User) and SMOOTH (Safer Medicines Outcomes on Transfer Home) are helping people to improve their conditions.
Participants have reported having more energy, fewer visits to hospital, fewer sick days and more motivation to make healthier choices.
John joined the Healthy Hearts Programme following coronary artery bypass surgery. He is now living smokefree, has a more heart healthy diet and is back working fulltime – which is doing wonders for his psychological wellbeing and sense of worth.
After attending the Better Breathing programme, Keniseli, who was often hospitalised due to breathing difficulties, can now mow his lawn without being short of breath and hasn’t been in hospital since 2011.
And Agnes, who was having trouble managing her medications at home is now doing well after the SMOOTH team helped her reorganise her medications into weekly blister packs.
