Background/Aim: “Respond to the first fracture, prevent the second” is the core function of Fracture Liaison Services. Counties Manukau has an estimated 1200 fragility fractures per year. In response to the growing burden of fragility fractures Counties Manukau Health has recognised the need for a systematic and coordinated approach to secondary fracture prevention. The aim is to develop and implement a Fracture Liaison Service to identify all patients over the age of 50 within the Counties Manukau region that suffer a fragility fracture. Once identified the service will ensure appropriate investigations are completed and appropriate treatment implemented. The Fracture Liaison Service will aim to improve quality of care and reduce the incidence of secondary fragility fractures.
Methods: Using the Institute of Health Improvement’s Model for Improvement a Fracture Liaison Service model of care was designed and implemented within Counties Manukau Health. Inclusion criteria included patients over the age of 50 with a fragility fracture presenting to secondary care at Counties Manukau Health from the 1st of February 2016. Exclusion criteria included fractures of the skull, feet and hands and any pathological fractures.
Results: TBC = Between 1st February 2016 and 30th May 2016 xxx new patients admitted to secondary care with a fracture were screened electronically by the Fracture Liaison Coordinator. xxx patients met the criteria for inclusion. xxx patients were given education on falls prevention, bone protection medication, lifestyle modification and osteoporosis. Xx patients were referred for a DEXA scan, xx for specialist assessment and xx recommended for immediate bone protection medication.
Conclusions: TBC = Fracture liaison services are effective in identifying fragility fracture patients. They are a viable option for coordinating the investigation and treatment of osteoporosis.
Future direction will Fracture Liaison Services will begin targeting patients in primary care. This will require collaboration between primary and secondary care services.
