Fiscal constraint is driving health systems around the world to work smarter. Could better integration of care improve clinical outcomes, patient experience and efficiency? How do we move a health system towards better integration? Quality improvement may be an answer. This session will describe two Australian quality improvement collaboratives which brought primary care, community and hospital professionals together to integrate care around complex high needs patients. The projects aimed to improve patient care while building inter-professional relationships, introducing technology and increasing regional quality improvement capacity.
