Congratulations to the Acute Care for the Elderly (ACE) team for producing and sharing their first health system improvement guide.
The guide describes the project the team undertook as part of Counties Manukau Health’s Beyond 20,000 Days campaign. It includes the problem the team addressed, their aim, the improvements they developed and tested, and the outcomes of their project.
The ACE team created a multidisciplinary geriatric unit at Middlemore Hospital to coordinate acute care with rehabilitation services.
The project reduced the average length of stay for patients in the target population from 25 days to 17 days and saved approximately 600 bed days. Saved costs from the project were estimated to be $309,010.
The health system improvement guide is a celebration of the ACE team’s hard work, and a learning tool for other health professionals wanting to adopt and implement the programme in their own areas.
