Congratulations to the Supporting Life After Stroke (SLAS) team for producing and sharing their first health system improvement guide.
The guide describes the project the team undertook as part of Counties Manukau Health’s Beyond 20,000 Days campaign. It includes the problem the team addressed, their aim, the improvements they developed and tested, and the outcomes of their project.
The SLAS team implemented a home-based specialist rehabilitation service for patients with mild to moderate stroke. The service provides early supported discharge for stroke patients, and is the first of its type in New Zealand.
SLAS saved 492 bed days over 10 months and improved the patient experience, achieving a 99% patient satisfaction rating.
The health system improvement guide is a celebration of the SLAS team’s hard work, and a learning tool for other health professionals wanting to adopt and implement the programme in their own areas.
