DOWNLOAD THE POSTERChronic obstructive pulmonary disease (COPD) and heart failure (HF) are two of the five most common long term conditions in Counties Manukau Health (CM Health). Pulmonary rehabilitation (PR) in CM Health is well established and evidence based. In comparison, evidence to support cardiac rehabilitation (CR) for people with HF is growing, with few resources allocated to meet the exercise needs of this growing population. Traditional ‘organ’ / ‘disease’ or service-based rehabilitation programmes are common, yet most participants have multiple co-morbidities. For example, in CM Health (2011 data),23% of the 4,490 people identified as having an admission related to HF, were found to also have COPD. Similarly, of the 5,620 people identified with COPD, 18% had HF. Diabetes and gout were found to be the most common other co-morbidities.
